Concierge Platinum Membership Terms and Conditions

  1. Membership Benefits:
    • The Platinum Concierge Membership provides you with:
      • Comprehensive annual visit – Once yearly with full review of health status, blood test results, metabolic health, hormone management and the latest in longevity science
      • Access to a private, secure chat line with Dr. Jila for health-related inquiries and follow-ups.
      • Same-day or next-day, unlimited appointments, either virtual or in-person, based on your preference and Dr. Jila’s availability.
      • Access to special live events, exclusive podcasts, and blogs tailored to women’s health and wellness.
      • Late-hour and early-morning chat support for non-urgent inquiries

  2. Access to Services:
    • The private chat line is available during regular business hours for non-urgent inquiries, with additional support available during late hours and early mornings. Messages sent outside of regular hours will be responded to as soon as possible.
    • Same-day and next-day appointments are subject to availability. While we strive to accommodate all requests, we recommend scheduling as early as possible to ensure your preferred time.
    • One time consultation/ single visits for non concierge members, whether in-person or virtual, are offered at a rate of $495. These visits are intended for introductory meetings and single focused problems.
    • In-person appointments are available for local patients only, and virtual appointments can be scheduled for patients located within permitted regions for telemedicine services.

  3. Limitations of Membership:
    • The Platinum Concierge Membership does not replace primary care or urgent/emergency care. In the event of an emergency, please call 911 or visit the nearest emergency room.
    • Services provided under this membership focus on women’s health, including perimenopause, menopause, hormone therapy management, and related concerns.

       

  4. Privacy and Confidentiality:
    • Your health information is confidential and secured in compliance with HIPAA regulations. All communications via the private chat line are encrypted and protected.
    • By joining the membership, you consent to the secure collection and storage of your data for the purpose of delivering concierge services.
