Key Takeaways from the WHI Hormone Therapy Study on Cardiovascular Biomarkers:
• Hormone therapy (HT) with both CEE and CEE+MPA led to improved lipid profiles—lower LDL and total cholesterol, with HDL levels increasing more in the CEE-alone group.
• Triglycerides went up in both groups, which is something to watch, since that can raise cardiovascular risk.
• Lipoprotein(a), another CVD risk marker, decreased—especially in women of American Indian, Alaska Native, Asian, or Pacific Islander backgrounds.
• Glucose and insulin sensitivity improved, which could be a good sign for metabolic health.
• Coagulation markers told a mixed story: increased factor VII (which may raise clot risk), but decreased fibrinogen (which could lower it). So, it’s complicated.But here’s the bigger picture:
• Despite these biomarker improvements, prior WHI trials didn’t show a drop in heart disease events—and early HT use was linked to a higher stroke risk.
• This study focused on oral HT. Other research (like KEEPS) suggests transdermal estrogen might act differently, especially on clotting and lipids.
• Bottom line: hormone therapy decisions should be personal, based on a woman’s individual risks, symptoms, and preferences.Looking ahead:
• We need more research on how different types and routes of HT impact heart health long-term.
• Understanding how HT affects clotting—and whether that translates to real-world outcomes—is still a work in progress.
• It’s also worth digging deeper into how HT works across diverse populations, especially with findings like the lipoprotein(a) drop in certain ethnic groups.
